DHS implements 24-month moratorium on new 245D license applications
Effective January 1, 2026 through December 31, 2027, DHS has paused new 245D home care license applications while it refocuses on providers not evaluated in over three years. Limited exceptions apply for high-need areas.
Minnesota EVV 80% compliance threshold takes effect July 1, 2026
Starting July 1, 2026, agencies must achieve at least 80% EVV compliance to receive full federal Medicaid matching dollars. DHS began monthly data reviews in January 2026 using post-payment review — no claim denials currently.
High-risk provider revalidation required by May 31, 2026
Minnesota launched statewide Medicaid provider revalidation on January 26, 2026. All high-risk service providers must complete revalidation by May 31, 2026, or risk disenrollment. A freeze on new provider enrollments in 13 high-risk service categories also took effect.
